Create Nursing Education wiki structure

New page

The NCLEX-PN is the National Council Licensure Examination for Practical Nurses, developed and administered by the National Council of State Boards of Nursing (NCSBN). Passing it is required for licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the United States. The examination measures whether a candidate can practice safely and effectively at the entry level.

This article is a general overview written for study purposes. Exam specifics change. NCSBN revises the test plan on a roughly three year cycle and periodically re evaluates the passing standard. For authoritative and current details, including exact content percentages, item counts, time limits, and item types, consult the official NCLEX-PN test plan and candidate bulletin published by NCSBN at ncsbn.org, and the requirements of your own board of nursing.

How the Exam Is Organized
The NCLEX-PN is not organized by nursing school course such as medical surgical or pediatrics. It is organized around Client Needs, four major categories, two of which are divided into subcategories. Content from all clinical specialties and all patient ages is distributed across these categories.

Integrated throughout every category are the integrated processes: the nursing process as applied at the practical nurse level, caring, communication and documentation, teaching and learning, culture and spirituality, and clinical judgment.

Safe and Effective Care Environment
Coordinated Care. The largest single subcategory on the PN exam. It covers collaboration with the interdisciplinary team, assignment and supervision of unlicensed assistive personnel, receiving and clarifying delegated tasks, advance directives, advocacy, informed consent, confidentiality and HIPAA, ethical practice, legal responsibilities and scope of practice, resource management, continuity of care and handoff, quality improvement, and the chain of command. See LPN Delegation and Reporting and LPN Scope of Practice.

Safety and Infection Control. Accident and injury prevention, fall precautions, standard precautions and transmission based precautions, surgical asepsis, handling hazardous and infectious materials, home safety, emergency response and disaster planning, ergonomic principles, reporting of incidents and errors, restraints and safety devices, and security plans.

Health Promotion and Maintenance
Growth and development across the lifespan, aging process, antepartum, intrapartum, and postpartum care, newborn care, family planning, expected physical findings, health screening and promotion, data collection techniques, self care, immunizations, and lifestyle choices.

Psychosocial Integrity
Coping mechanisms, crisis intervention, grief and loss, end of life care, mental health concepts, behavioral interventions, therapeutic communication and therapeutic environment, chemical and other dependencies, abuse and neglect, cultural awareness, religious and spiritual influences, sensory and perceptual alterations, stress management, and support systems.

Physiological Integrity
Basic Care and Comfort. Assistance with activities of daily living, mobility and immobility, personal hygiene, nutrition and oral hydration, elimination, rest and sleep, nonpharmacological comfort measures, and palliative care.

Pharmacological Therapies. Expected actions and outcomes, adverse effects and contraindications, medication administration, dosage calculation, pharmacological pain management, and, within PN scope, the monitoring aspects of parenteral and intravenous therapies. Note that the exam tests the PN scope boundary here, so it commonly asks what the LPN may and may not administer. See LPN Skills and Practice Limitations.

Reduction of Risk Potential. Recognizing changes in vital signs, laboratory values, and diagnostic test results, potential for complications from procedures and health alterations, therapeutic procedures, and monitoring of conscious sedation and other treatments within scope.

Physiological Adaptation. Alterations in body systems, basic pathophysiology, fluid and electrolyte imbalances, care of the patient with a medical emergency, unexpected responses to therapies, and radiation therapy.

Approximate Content Distribution
The following percentage ranges are those published in the 2023 NCLEX-PN test plan and are reproduced here as an illustration of relative weighting. Verify current percentages against the test plan in effect on your examination date.

Client Needs category !! Subcategory !! Percentage of items
rowspan="2" | Safe and Effective Care Environment || Coordinated Care || 18 to 24 percent
Safety and Infection Control || 10 to 16 percent
Health Promotion and Maintenance || || 6 to 12 percent
Psychosocial Integrity || || 9 to 15 percent
rowspan="4" | Physiological Integrity || Basic Care and Comfort || 7 to 13 percent
Pharmacological Therapies || 10 to 16 percent
Reduction of Risk Potential || 9 to 15 percent
Physiological Adaptation || 7 to 13 percent

Two observations for planning study time. First, Physiological Integrity as a whole is the largest area, accounting for roughly a third to more than half of the examination when its four subcategories are combined. Second, Coordinated Care is the largest single subcategory, which is why scope of practice, delegation, supervision, and prioritization deserve concentrated study.

Next Generation NCLEX
NCSBN launched the Next Generation NCLEX (NGN) in April 2023 for both the RN and PN examinations. The change was driven by evidence that entry level nurses make a high proportion of practice errors involving clinical judgment rather than knowledge recall.

The NCSBN Clinical Judgment Measurement Model describes six cognitive steps that NGN items are designed to measure:

# Recognize cues. Identify which findings are relevant and significant.
# Analyze cues. Link those findings to the patient's condition.
# Prioritize hypotheses. Rank the most likely and most urgent explanations.
# Generate solutions. Identify possible actions and desired outcomes.
# Take action. Implement the highest priority interventions.
# Evaluate outcomes. Compare observed outcomes to expected outcomes.

At the practical nurse level these steps are applied within the PN scope, so the expected action is frequently to collect focused data, report to the RN, or implement an intervention within scope rather than to redesign the plan of care.

The NGN PN examination uses stand alone clinical judgment items, which present a single clinical scenario followed by one item. Two commonly discussed formats are the bow tie item, which asks the candidate to place the condition, the actions to take, and the parameters to monitor into a bow tie shaped diagram, and the trend item, which presents data over several time points and asks the candidate to interpret change. The multi part unfolding case studies are a feature of the NCLEX-RN rather than the PN examination.

Item Types
Candidates should expect a mix of traditional and newer formats:

* Multiple choice, four options and one correct answer. This remains the most common item type.
* Multiple response, select all that apply, and newer variants such as select N, select all that apply on a table or grid, and matrix items in which the candidate classifies each finding across columns.
* Fill in the blank calculation, most often a dosage, an intravenous flow rate, or an intake and output total.
* Ordered response or drag and drop, placing steps of a procedure or priority actions in sequence.
* Hot spot, clicking a location on an image such as an auscultation site.
* Chart and exhibit items requiring the candidate to review a medical record before answering.
* Cloze or drop down items in which the candidate selects words from menus to complete a sentence about the patient.
* Highlight items in which the candidate selects the relevant words or phrases in a passage of text.
* Audio and graphic items, which appear less frequently.

Some NGN item types use partial credit scoring, so answering part of an item correctly can earn partial credit. Traditional items are scored right or wrong. There is no penalty for guessing, and every item must be answered before the exam advances. Candidates cannot skip an item or return to a previous one.

Format and Administration
* Computerized adaptive testing (CAT). The computer selects each item based on the candidate's performance on prior items, targeting items near the candidate's ability level. Answering correctly leads to a more difficult item, answering incorrectly leads to an easier one. The result is an efficient and precise estimate of ability.
* Variable length. Under recent test plans the NCLEX-PN ranges from a minimum of about 85 items to a maximum of about 150 items, including a number of unscored pretest items that are being evaluated for future use. Candidates cannot tell which items are pretest.
* Time limit. Approximately five hours in recent administrations, including the tutorial and all breaks. Confirm the current limit in the candidate bulletin.
* Pass or fail decision rules. The exam ends when the computer is 95 percent certain the candidate's ability is clearly above or clearly below the passing standard, or when the maximum number of items or the time limit is reached. When time expires or the maximum length is reached without that certainty, alternate rules based on the final ability estimate apply.
* Scoring. The result is pass or fail only. There is no numerical score and no percentage. Candidates who fail receive a Candidate Performance Report showing performance relative to the passing standard in each content area.
* Passing standard. NCSBN sets a criterion referenced passing standard on a logit scale and re evaluates it on a regular cycle. Because the standard can change, check the current value with NCSBN.
* Registration and administration. Candidates register with NCSBN through Pearson VUE and apply for licensure with a board of nursing. After the board declares eligibility, the candidate receives an Authorization to Test and schedules at a Pearson VUE test center.
* Retakes. A candidate who fails may retest after a waiting period defined by NCSBN policy and by the board of nursing, commonly 45 days between attempts, with a limit on attempts within a twelve month period. Boards may impose additional requirements.

The exam length tells you nothing about the outcome. A candidate may pass or fail at 85 items or at 150. Continuing past the minimum simply means the estimate is still near the passing standard.

Study Strategy
Build the plan around the blueprint
* Allocate study time by category weight, not by how comfortable the content feels. Coordinated Care and Pharmacological Therapies together represent a large share of items.
* Convert the test plan outline into a checklist and track coverage rather than hours.
* Study across the lifespan and across settings within each category, since the exam is not organized by course.

Think like the exam
* Assume the ideal setting. Adequate staffing, all supplies available, and an RN and provider reachable. Answer according to textbook standard, not the shortcuts used at a clinical site.
* Answer within PN scope. If an option describes an initial assessment, a nursing diagnosis, care plan development, IV push, blood administration, or initial teaching, it is usually wrong for the LPN.
* Prioritize. Use airway, breathing, circulation, then safety, then Maslow's hierarchy with physiologic needs before psychosocial. Prefer the acute, new, unstable, or unexpected finding over the chronic and expected one.
* Prefer assessing or collecting data before acting, unless the patient is in immediate danger or the stem already gives complete data, in which case act.
* Do not leave the patient. Answers that involve leaving an unstable patient to find someone are usually wrong when another option provides help without leaving.
* Least restrictive and least invasive first for restraints and for many interventions.
* Therapeutic communication answers acknowledge feelings, stay open ended, and avoid false reassurance, advice giving, and asking why.
* Read the stem for the actual question. Watch for words like first, best, priority, initial, most important, and for negatively worded stems such as which action requires intervention or which statement indicates a need for further teaching.

Practice deliberately
* Do large volumes of practice questions and, critically, read the rationale for every option, including the ones you got right.
* Keep an error log grouped by cause: knowledge gap, misread stem, priority error, or scope error. Patterns show up quickly.
* Practice NGN formats specifically, including matrix, cloze, highlight, and bow tie items, since they read differently from classic multiple choice.
* Take full length timed practice tests to build stamina for a five hour session.
* Master dosage calculation until it is automatic, including conversions, weight based dosing, reconstitution, and intravenous flow rates.
* For pharmacology, learn drug classes, suffixes, mechanisms, key adverse effects, and required monitoring rather than memorizing individual brand names.
* Learn laboratory reference ranges and the critical values that require immediate reporting.

Practical matters
* Use current preparation materials aligned to the test plan in effect on your exam date.
* Verify identification requirements, arrival time, and test center rules in the candidate bulletin before exam day.
* Plan sleep, nutrition, and break strategy. Fatigue late in a long adaptive exam is a real source of lost items.

Official Sources
Because exam specifics change, treat the following as authoritative rather than any secondary summary, including this article:

* The current NCLEX-PN Test Plan published by NCSBN, which lists every content category, subcategory, and the current percentage ranges.
* The current NCLEX Candidate Bulletin from NCSBN, which covers registration, scheduling, identification, item counts, time limits, and retake policy.
* Your own state board of nursing, for licensure application requirements, fingerprinting and background check rules, and any state specific conditions.
