=== HU-072: Clinical placement documentation sent to affiliated hospitals without student name, causing hospitals to not respond; institution then blamed hospitals for non-response rather than acknowledging the documentation error === {| class="wikitable" style="width:100%" ! style="width:12em" | Severity | High ! style="width:12em" | Status | CONFIRMED |- ! Issue type | Institutional Misrepresentation about Clinical Outreach Efforts ! Date added | 2026-05-05 |- ! Owner | colspan="3" | Clinical Placement / Program Chair / Compliance |} '''Finding.''' Multiple students reported that Herzing's clinical placement office told them affiliated hospitals did not respond to placement requests, when in fact Herzing had sent placement documentation without the student's name listed, preventing hospitals from identifying or responding to the request. When students contacted hospitals directly, hospital staff confirmed they never received an identifiable student request, because Herzing had been unable to include the student's name in what it sent. The institution shifted blame to the hospitals rather than acknowledging the documentation error. '''Evidence.''' Facebook post by Student N, August 13, 2024: Baptist has an active affiliation with Herzing, and even that did not stop the institution from sending Student N far away; the school said Baptist did not respond, but another student described in the post as a girl said the documentation was sent with no student name at her hospital of choice, so the hospital did not respond; Student N wondered whether the same explained Student N's own case and said Student N was trying to fight it for the next semester, adding that Student N loves this site. Corroborating post by Student F, May 25, 2024: clinical placement is not reliable, and the office seems to have a standard of telling students it reached out to hospitals, when contacting the hospital directly reveals the hospital never received an email. Source Group: Herzing Direct Entry MSN Facebook Group Export (Dec 2022 to May 2026), Topical/Keyword Review (Volume 6). '''Reported statement.''' Student N, a student pseudonym, paraphrased that Baptist has an active affiliation with Herzing but that did not prevent a distant placement, that the school claimed Baptist did not respond, and that another student, described as a girl, learned documentation sent with no student name explained a hospital's non-response, leaving Student N wondering whether the same happened in Student N's own case and stating Student N was trying to fight it for next semester. Student F, a separate student pseudonym, paraphrased that clinical placement is not reliable and follows a pattern of claiming outreach was made to hospitals when, once a student contacts the hospital directly, the hospital says it never received an email. '''Standards.''' 34 CFR 668.71 to 75 (misrepresentation) provides that institutions may not make substantial misrepresentations to students about the nature of services they provide. Telling a student that an affiliated hospital did not respond, when in fact the institution sent inadequate documentation, constitutes a misrepresentation about the institution's clinical outreach efforts. ACEN Standard 5.1 treats clinical placement as an institutional responsibility, and misrepresenting placement efforts harms student academic progression. The Federal Trade Commission Act Section 5 (deceptive practices) requires that institutional statements to enrolled students about placement efforts be truthful and substantiated. '''Impact.''' Students relied on the institution's representation that hospitals did not respond and accepted distant placements as the only option. Had they known the actual cause was inadequate institutional documentation, they could have demanded the placement be re-attempted with proper documentation. The pattern wastes student time, forces unnecessary travel costs (see {{HU|HU-024}}, {{HU|HU-049}}), and shifts institutional responsibility for placement failures onto affiliated hospitals. Multiple students in the same cohort identifying this same pattern across different placement attempts indicates a systemic institutional documentation practice rather than an isolated error. '''Notes.''' This issue substantially expands {{HU|HU-007}}, {{HU|HU-035}}, {{HU|HU-038}}, and {{HU|HU-049}} by identifying a specific institutional practice, incomplete documentation, that may be the root cause of many reported placement failures. It also intersects with {{HU|HU-042}} (hospital reportedly refusing Herzing students). Recommended verification should include: an audit of placement requests sent to affiliated hospitals to determine whether student names were consistently included; an interview of the clinical placement coordinator about documentation practices; and hospital partner interviews about communications received from Herzing. The Student F corroboration, from a different student in a different cohort, strengthens the finding: the practice is documented across multiple cohorts.
