== Overview == This article gathers 32 tracker entries dated from April 30, 2026 through June 15, 2026. All 32 are CONFIRMED except one lettered sub-issue, {{HU|HU-120c}}, which is marked POSSIBLE pending verification, and the {{HU|HU-169}} cluster of four entries, which is recorded evidence-first as a reported and unverified corroborating account. By severity, the set includes 4 Critical entries, 22 High entries, and 6 Medium entries. The Critical entries are the Spring 2025 mass grade transfer crisis ({{HU|HU-082}}) and the three-part Canvas/Instructure ShinyHunters breach cluster ({{HU|HU-120}}, {{HU|HU-120a}}, {{HU|HU-120b}}). Three recurring threads run through the set. The first is chronic unreliability in the Elsevier-owned academic platforms that carry most of the MSN-DE coursework: Sherpath's repeated failures to load lessons, preserve completed work, and transfer grades to Canvas ({{HU|HU-041}}, {{HU|HU-075}}, {{HU|HU-082}}, {{HU|HU-129}}); its predecessor Realizeit's grade-recording and exam-format problems ({{HU|HU-076}}, {{HU|HU-076a}}, {{HU|HU-076b}}); and the Elsevier Adaptive Quizzing (EAQ) content and grade-conversion errors ({{HU|HU-100}}). The second is a cluster of NSG525 Week 1 content and remediation failures in May 2026, including duplicate quiz answer choices, incorrect clinical units, empty required assignments, and a burden-shifting remediation ({{HU|HU-112}}, {{HU|HU-112a}}, {{HU|HU-112b}}, {{HU|HU-112c}}, {{HU|HU-122}}). The third, and most extensively documented, is the June 2026 NSG523 HESI Exam 1 score-transmission failure: a completed exam invisible to the instructor in Elsevier's Evolve system for over 51 hours, a 0/100 grade and a 69-percentage-point course grade swing, a resolution that depended on the student producing his own proof, and two successive IT closures that declined to identify a root cause ({{HU|HU-168}} through {{HU|HU-173}}), corroborated by a second student's account of the same failure pattern across two other courses ({{HU|HU-169}} cluster). Ownership recurs across the IT Department, Academic Affairs, and vendor management for Elsevier and Sherpath, with individual entries also naming Instructure, the CISO, named adjunct faculty (Dr. Tanetshia Brown, Dr. Ashley Jennings, Dr. Heather Gregg, Dr. Stacey Gombar), and the IT Service Desk (Andrew Cox). Two entries fall outside the platform-reliability and grading pattern: {{HU|HU-001}}, an enrollment-side Canvas provisioning gap at the start of the program, and {{HU|HU-043}}, the loss of a clinical instructor and course announcements on the first day of Clinical IV.
